The Napa/Solano Area Agency on Aging, Disability Services & Legal Center, and Providence Community Health Napa Valley (provider of Napa County’s Information & Assistance) proudly announce the launch of Connections Napa County, the Aging and Disability Resource Connection (ADRC) for Napa County.

Connections Napa County will become the central hub for older adults, individuals with disabilities, and caregivers seeking current, reliable information about the programs and services available in our community, to help them live healthy, happy, independent lives.

Connections will provide a one-stop-shop website containing information about healthcare, housing, food, transportation, caregiver support, and more. A Connections Information & Referral Team will be available to answer questions and provide guidance to individuals and families so they can make informed decisions, help expedite and coordinate urgent/critical services when needed and assist with transitions from hospitals or facilities to home. As part of the ADRC model, existing community-based organizations will work together to provide a more person-centered, integrated, and efficient support system across the County.

“Our community is aging. Napa County is home to a higher-than-average older adult population,” says Elaine Clark, Napa/Solano Area Agency on Aging Executive Director. “I believe that Connections Napa County will become the cornerstone for all of us who need or provide services for these populations. It will make it easier for people to navigate complex systems and connect with the support they need. At the end of the day, our goal is to keep people thriving in the community.”

In 2024 Providence Community Health, in partnership with Napa County Health & Human Services, conducted the Napa County Older Adults Assessment (NOAA). The assessment's findings highlighted a significant need for a coordinated approach to service delivery that was easy to access and user friendly. This finding underscored the need to create an ADRC for Napa County.

The California Department of Aging and the Department of Rehabilitation approved Connections Napa County as an Emerging ADRC. The next step is to develop the necessary infrastructure and strategic partnerships to become a dedicated ADRC by Summer 2025.

The inception of ADRCs began as a collaborative initiative by the Administration for Community Living (ACL) and the Centers for Medicare & Medicaid Services (CMS) in the early 2000s to serve as a single-entry point for all long-term services and supports (LTSS). In 2021, they became an integral part of California’s Master Plan for Aging to prepare the state for its growing aging population. There are currently 25 ADRCs operating across California.

California’s ADRCs are led by a core partnership between Independent Living Centers (Disability Services & Legal Center) and the Area Agencies on Aging (Napa/Solano Area Agency on Aging). Providence Community Health Napa Valley was chosen as a third core partner given their long history of providing services for older adults and Information & Assistance which is a core service of an ADRC.
