Our agency offers a continuum of care for individuals and families, tailoring teams of professionals – which may include physicians, nurses, social workers, volunteers, spiritual counselors, and occupational, physical, and speech therapists – to address the needs of each individual and family.
Our spectrum of services is offered through two divisions – Community Health Napa Valley and Hospice Napa Valley – and four service lines. Community Health Napa Valley offers Alzheimer’s services, our award-winning Day Program, and Palliative Services; Hospice Napa Valley offers end-of-life care and bereavement services. Hospice: Bilingual Bereavement Coordinator
The Program Coordinator, reporting to the Supervisor of Bereavement and Social Work, is responsible for coordinating the community-based bereavement support program. Responsibilities include but are not limited to initial intake, volunteer engagement, planning, coordinating and scheduling of support groups and programs/activities, referrals and community resources, and administrative tasks such as data management and program correspondence.

PACE: Physical Therapist
The Physical Therapist (PT) is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The PT assumes responsibility for participants treated by the PTA and PT aide.
