Why ECM?

Medi-Cal members typically have several complex health conditions involving physical, behavioral, and social needs. Members with these complex needs must often engage several delivery systems to access care, including primary and specialty care, dental, mental health, substance use disorder treatment, and long-term services and supports.

 

In fact, more than half of Medi-Cal spending is attributed to the 5 percent of members with the highest-cost needs. Through enhanced care management, we aim to provide care coordination across healthcare delivery systems to reduce barriers associated with multiple complex health conditions and social circumstances.

Who is eligible for ECM?

  • Adults who are at risk for avoidable hospital or emergency department care.
  • Adults with serious mental health and/or substance use disorder needs.
  • Adults living in the community and at risk for long-term care or institutionalization.
  • Adult nursing facility residents transitioning to the community.
  • Must be 21+
  • Must have Partnership Health Plan

Your ECM Lead Care Manager can:

  • Help you get health care and social services
  • Find and schedule your health visits
  • Arrange transportation to and from health visits
  • Connect you to programs and services in your community
  • Explain your doctor’s prescription and how to use them
  • Refer you to mental health services
  • Assist with returning home from a nursing home
  • Provide support for drug and alcohol misuse
  • Provide housing support
