Enhanced Care Management Program (ECM)

Helping you achieve your health goals!

“ECM was designed to help members of the community with the highest, most complex medical and social needs. We take a person-centered and interdisciplinary approach to meet our clients where they are—in life, in their health journey, and in their community. Our goal is to meet both the clinical and non-clinical needs of our members by considering the impacts of ALL drivers of health—physical, mental, and social.”

Why ECM?

Medi-Cal members typically have several complex health conditions involving physical, behavioral, and social needs. Members with these complex needs must often engage several delivery systems to access care, including primary and specialty care, dental, mental health, substance use disorder treatment, and long-term services and supports.

In fact, more than half of Medi-Cal spending is attributed to the 5 percent of members with the highest-cost needs. Through enhanced care management, we aim to provide care coordination across healthcare delivery systems to reduce barriers associated with multiple complex health conditions and social circumstances.
