Clinical Application Specialist

We are seeking a full-time, exempt Clinical Application Specialist to be the vital link between our information technology capacity and our business objectives by supporting and ensuring the successful completion of analytical, building, testing and deployment tasks of our clinical information systems.

Village Coordinator

The Village Program Coordinator reports to the Director of Operations and provides ongoing care coordination, case management, and other therapeutic services to Upvalley Village members.  The Village Program Coordinator is responsible for creating programming and services that enable older adults to enjoy a vibrant life through volunteering, connecting with neighbors, accessing trusted programs, community resources, and vetted services.  The Village Program Coordinator has primary responsibility for promoting the Upvalley Village to the community and for working with the Director of Operations and Advisory Council to create new programming and secure ongoing funding for program sustainability.

Medical Records Manager

Our Medical Records department is seeking a full-time manager to be responsible for managing all aspects of maintaining and protecting the organization’s clinical medical records, including the management of the clinical records team.

Hospice Clinical Services Manager

Our Hospice department is seeking a full-time Hospice Clinical Services Manager, to collaborate with other Hospice Interdisciplinary Team Leaders to promote the provision of quality Hospice care in all settings.  Has direct supervisory oversight of the Hospice disciplinary team.  Candidate must be capable of providing ongoing staff, volunteer, departmental, and aide community education.

Performance Improvement Coordinator

We are currently seeking a full-time Performance Improvement Coordinator, professional registered nurse responsible in maintaining and revising the Performance Improvement Programs for Hospice, Palliative Care, and Adult Day Services. Coordinates the activities of departments, committees, and staff to develop and document ongoing evaluation activities, perform studies and problem focused reviews, interpret data and promote the Performance Improvement Program to clinical and support services. Assists the Director of Performance Improvement and may assume the role when necessary.

Program Director –  Collabria Day Program

Collabria Care is comprised of a team of dedicated staff and volunteers committed to providing exceptional professional healthcare, education, and service to the community. We provide a caring presence as we seek to comfort the whole person and family—body, mind, and spirit—through chronic conditions, illness, death, and bereavement.
We are currently seeking a Program Director that coordinates, implements, and supervises all services of the Collabria Day Program, to follow and interpret all policies and procedures, applicable laws and licensing requirements at all times, regarding ADHC, ADP and ADCRC programs. The Director leads all strategic planning program development, quality, community outreach and Person-Centered Care initiatives for the Collabria Day Program. This position reports to the President/CEO.
