What is PACE?

The PACE (Program for All-inclusive Care for the Elderly) model of care is based on the on the belief that it is better for the well-being of older adults with chronic care needs, and their families, to be cared for in their community whenever possible.

PACE delivers comprehensive medical and social services to older adults through personalized plans of care, created by an Interdisciplinary CareTeam, that address all preventative, primary, acute and long-term care service needs. This enables each participant to remain independent in their home for as long as possible.

The focus is on you, the participant.

Why is PACE a great option for older adults?

Our goal is to help you stay as healthy and independent as possible. Your Interdisciplinary Care Team is experienced in caring for older adults and will create a healthcare plan that is personalized to meet your needs.

We’re also here to assist you in scheduling appointments, filling out paperwork, and will provide transportation to all medical appointments if you need it.

We closely monitor your health--lowering your chance of experiencing a health crisis and leading to better overall health.

Who Qualifies?

PACE is available to anyone who:

  • is age 55 or older
  • living in our service area
  • needs nursing home-level of care*
  • is able to live safely in the community

* As determined by the California Department of Health Care Services

Service Area

Providence PACE serves most of Napa County, parts of Solano County, and the city of Sonoma.

What's the Cost?

Providence PACE is a managed care health plan.

We accept Medi-Cal, a combination of Medi-Cal and Medicare (dual eligible), Medicare, or private payment.

For those with Medi-Cal and no-share-of-cost, our PACE services are FREE. Medi-Cal beneficiaries who have a share-of-cost remain responsible for their individual share-of-cost.

Participants without Medi-Cal pay a monthly premium to cover long-term care and Medicare Part D drugs.

There are no deductibles, copayments, or coinsurance for any care and services approved by our team of healthcare professionals.

Respect and Non-discrimination
Each participant has the right to considerate, respectful care from all Providence PACE employees and contractors at all times and under all circumstances. Each participant has the right not to be discriminated against in the delivery of required services or treated differently because of race, color, national origin, religion, age, disability, sex, sexual orientation, gender identity and expression, or based on any other classification protected under applicable local, state, or federal laws.

Information Disclosure
You have the right to receive accurate, easily understood information and to receive assistance in making informed health care decisions.

To contact 1-800-MEDICARE for information and assistance, including to make a complaint related to the quality of care or the delivery of a service

Choice of Providers
You can choose a primary care provider from within the Providence PACE network that is no more than 15 miles from your home, and specialists from within our contracted network.

Reasonable and timely access to specialists as indicated by the participant’s health condition and consistent with current clinical practice guidelines.

Receive necessary care in all settings, up to and including placement in a long-term care facility when the PACE organization can no longer provide the services necessary to maintain the participant safely in the community.

Access to urgent and emergency services when and where the need arises.

Participation in Treatment Decisions
You or your designated representative have the right to participate fully in all decisions related to your treatment.

Confidentiality of Health Information
You have the right to communicate with health care providers in confidence and to have the confidentiality of your health care information protected. You also have the right to review, copy, and request amendments to your own medical records.

Grievance and Appeals
You have the right to a fair and efficient process for resolving differences with Providence PACE.

A complete copy of the PACE Participant Bill of Rights is available upon request. Please ask your Providence PACE care team.
