Community Health Napa Valley is the only nonprofit (501c3) provider of hospice services, based in the Napa Valley, where we have served for nearly four decades. Our agency offers a continuum of care for individuals and families, tailoring teams of professionals – which may include physicians, nurses, social workers, volunteers, spiritual counselors, and occupational, physical, and speech therapists – to address the needs of each individual and family.
Our spectrum of services is offered through two divisions – Community Health Napa Valley and Hospice Napa Valley – and four service lines. Community Health Napa Valley offers Alzheimer’s services, our award-winning Day Program, and Palliative Services; Hospice Napa Valley offers end-of-life care and bereavement services. Driver - PACE
This position is responsible for providing safe transportation to participants in a timely, competent manner, and for communicating important information relevant to the overall care plan to the multidisciplinary team.
Constant Care Attendant - PACE
This position requires traveling to see patients in Napa County.
The Person-Centered Care Specialist creates and develops cognitive and sensory programs in the three activity spheres of Productivity, Self-Care, and Leisure to engage the participants in the Collabria Day Center. Facilitates individual activities to complement the uniqueness of each person. Uses a person-centered approach to maintain a healthy social environment for the adults.
Bilingual in Spanish/English strongly preferred. - Bi-Lingual (English/Spanish)
This is a per diem/on call position with occasional night shift coverage as needed.
The Hospice Registered Nurse provides prescribed nursing care service to Hospice patients and their families in a home setting, Respite Care Facility (RCFE) or a skilled nursing facility setting.
